All4One Voluntary Recall Form

ISSUE

Evenflo is voluntarily recalling certain models of the All4One 4-in-1 convertible car seat manufactured between January 2022 and June 2024 that were sold in the United States and Canada. See here for more detail or fill out the form below to request a replacement.

Note: The model name, model number, date of manufacture and serial number (if applicable) are found on a small white label on your product. With the car seat in recline position 6, look at the back of the car seat. The small white label will be facing towards the interior of the car seat on the left side. Example:
